The Anne Carlsen Center is dedicated to helping children and adults of all ages and their families build home lives of dignity, freedom and well-being. Our highly-trained staff equips individuals to thrive in their homes and in their community. ACC staff provides services in the home through customized supports by nurturing abilities, teaching life skills of independence, and measuring progress through a series of goals and objectives. ACC also teaches critical skills and routines to families and caregivers.

Services Offered

Individualized Supported Living Arrangement (ISLA) Services

Anne Carlsen offers services for adults 21 and over who desire to live in their own apartments, while receiving support and supervision to maintain health and safety. Our highly-trained staff equips individuals to thrive in their home, neighborhood and community. Staff provides services in the individual’s own home through customized supports by nurturing abilities, teaching life skills of independence, and measuring progress through a series of goals and objectives. Supports will be person-centered and based on each individual’s unique strengths, needs, hopes and dreams.

  • Support and strategies to enhance the individual’s control and independence over the environment, resources, activities and self-care
  • Focus on skill development through implementation of goals and objectives
  • Maintain continuity with team planning across all settings
  • Establish and monitor skill development objectives in behavioral, life and social skills

  • Special assessment in areas of need
  • Safety and health monitoring and maintenance

Behavioral Services

Anne Carlsen offers opportunities for individuals, families and their caregivers to achieve maximum growth and learning. Our services are tailored to help maximize abilities and enable individuals to successfully reside in a community setting. Each individual’s plan will be a reflection of his or her unique personal interests, strengths and dreams.

  • Initial observation
  • Assessments of current intervention for effectiveness
  • Development of person-centered intervention plan with outcomes and training for family, caregivers and other natural supports
  • Observing and collecting data for interventions
  • Documentation, review and revision of plan
  • Providing training and technical assistance to relevant individuals
  • Participation in team meetings

These services are scheduled directly with the family and are carried out in the family home and community. A behavior support coordinator will provide a plan of intervention and training for the individual and caregivers to address behavioral challenges within the actual context, and with specific goals in mind. It is important for individuals to increase their skills in self-management with family and caregivers providing consistent support.
